Major energy and industrial losses have been investigated, including mercury attack of an gas separation plant at PTT, Thailand; reformer failures in Malaysia, Indonesia and Thailand; a petrol pipeline fire at a marine terminal in Penang; corrosion and fatigue failures of tube bundle, flat plate and spiral heat exchangers, both aqueous and oil ; the collapse of chimneys, storage systems and FRP tanks ; gear failures in cement, sugar, metallurgical and fertiliser industries; rotary kiln and drier failures; and numerous boiler failures in Malaysia, Thailand and Indonesia, including a black liquor boiler in Sumatra.
